I, Shaik Khadar Basha S/o Shaik Chand Basha, residing at H.No: 28-1052-63-1, Devanagar, Nandyal-518501, Nandyal District, Andhra Pradesh, hereby inform that my Indian Original Passport bearing No. T2511982, issued at Vijayawada, has been lost. The passport was lost on 08-06-2026 at around 11:00 AM while I was travelling by motor cycle from my residence towards Nandyal MRO Office. The hand bag containing the passport and some other important documents was also lost between my house and Girinath Center.
