I, NB/Sub Mehender Kumar, Army No. JC-711081-X, serving in Unit 426 Field Hospital, hereby state that my son’s name has been recorded as “Vipin Kumar” in my service records as well as in his 10th and 12th educational certificates. In all future records and official documents, my son shall continue to be known as Vipin Kumar.
