I, Lalita Kumari, W/o Sheokinder, R/o House No. 224, 2nd Floor, Police Line, Near City Thana, Gurgaon, Haryana-122001, hereby declare that I have changed the name of my minor son aged 14 years from his earlier name to “Hardik Chauhan”. Henceforth, he shall be known as Hardik Chauhan for all future records and purposes.
