I, Memebub Hossain, son of Majir Uddin Miya, resident of Vill. Gokuler Kuthi, P.O. Rashidanga 2, P.S. Kotwali, District Cooch Behar, West Bengal, hereby declare that my father’s name has been changed from “Mazir Uddin Ahamed” to “Majir Uddin Miya” through an affidavit sworn before the Ld. 1st Class Judicial Magistrate, Cooch Behar vide Affidavit No. 28AC631450 dated 24/06/2026. It is further declared that “Mazir Uddin Ahamed” and “Majir Uddin Miya” are one and the same person, and all records should be updated accordingly. West Bengal
