We, Shazia Asthana and Rajat Asthana, residents of H-32A, Top Floor, Kalkaji, New Delhi, declare that in the birth certificate of our daughter Samayra Asthana, the father's name should be corrected from “Rajat” to “Rajat Asthana” and the mother's name from “Shalu” to “Shazia Asthana”. Both names refer to the same persons.
