It is hereby declared that Shri Manojit Modak, S/o Sashi Bhusan Modak, residing at Debnagar, Jalpaiguri-735102, has changed his name and shall henceforth be known as Monojit Modak, as declared before the First Class Judicial Magistrate, Jalpaiguri vide Affidavit No. 13711 dated 22 May 2026.
