I, Paritosh Kant Sinha, resident of Flat No. 2503, 25th Floor, Tower-04, Panchsheel Greens-2, Plot No. GH-01A, Sector-16, Greater Noida, hereby declare that I have changed my name from “Paritosh Kant Sinha” to “Paritosh Sinha” and henceforth I shall be known as “Paritosh Sinha” for all future purposes.
