I, Yaddal Balakrishna (old name), resident of Raja Street, Kanuparthipadu, Nellore-524004, SPSR Nellore District, Andhra Pradesh, hereby declare that I have changed my name and surname from Yaddal Balakrishna to Yeddala Balakrishna Reddy (new name), S/o Yeddala Subbarayulu.
