I, Yaddala Subbareddy (old name), resident of Raja Street, Kanuparthipadu, Nellore-524004, SPSR Nellore District, Andhra Pradesh, hereby declare that I have changed my name and surname from Yaddala Subbareddy to Yeddala Subbarayulu (new name), S/o Yeddala Sankar Reddy. From now onwards, I will be known as Yeddala Subbarayulu for all official and legal purposes.
