Jorebunglow, District Darjeeling, West Bengal, declares that his old name “Sahnkar Rai, S/O Jai Prakash Rai” has been officially changed to “Shankar Rai, S/O Jai Prasad Rai” as per affidavit sworn before the Learned Judicial Magistrate at Darjeeling vide affidavit serial no. 01 dated 23/06/2026. Both names are declared to belong to the same and identical person.
