I, MAHAVEER JAIN, resident of 27-12-35, Ali Beig Street, Governorpet, Vijayawada, NTR District – 520002, Andhra Pradesh, hereby declare that I have changed the name of my minor child from RIYA JAIN (Old Name) to RIYA JAIN (New Name), daughter of MAHAVEER JAIN. Henceforth, my minor child shall be known as RIYA JAIN for all legal and official purposes.
