Nima Lakki Bhutia, wife of Late Thendula Bhutia, residing at 9th Mile Lamahatta, Tukdah Tea Garden, PO Lamahatta, PS Rangli Rangliot, District Darjeeling-734213, West Bengal, hereby declares that her name was wrongly recorded as “Nima Lucky” instead of the correct name “Nima Lakki Bhutia”. She confirms that both names refer to the same and identical person and states that henceforth she shall be known as Smt. Nima Lakki Bhutia for all official and legal purposes, as per Affidavit No. 8416 dated 23-06-2026 sworn before the Executive Magistrate, Darjeeling.
