Smt. Sashi Lama, wife of Santa Kumar Lama, residing at K.B. Road, Ghoom Bhanjyang, PO Ghoom, PS Jorebunglow, District Darjeeling-734102, West Bengal, hereby declares that she has changed her name from “Sashi Lama” to “Sashi Lama Moktan”. She confirms that both names belong to the same and identical person and that henceforth she shall be known as Sashi Lama Moktan for all purposes, as per Affidavit No. 17857/2 dated 23-06-2026 sworn before the Notary Public at Darjeeling.
