Abu Sayeed Ali, son of Late Saleman Hoque, resident of Vill. Putkibari, PO Garalbari, PS Kotwali, District Jalpaiguri, West Bengal, hereby declare that my father’s name has been wrongly recorded as “Saleman Hoque” in my voter card and other documents, whereas the correct name is “Soleman Ali”. It is confirmed that “Saleman Hoque” and “Soleman Ali” refer to the same and identical person as per records including voter list (2002), Part No. 105, Sl. No. 397.
